Click on the link for the appropriate form for your application. Once the form appears in your web browser, print out the application form. Using the printed application form, fill in the information with a typewriter, or print clearly by hand. At the top of the page, fill in the fee amount for services requested and the total amount (in U.S. dollars). Please note that the Company Application applies to Immigration Attorneys and Businesses. All others should use the Individual Applicant form. Next, fill out the application section of the form. All information asked for is required in order to process your application.
